Info from Licensor: "This is what happens when you lose concentration for one second. I was training early in the mornings, got distracted by something on the walls and clipped the auto belay (usually clipped into a big triangle at the base) around the side of the wall. Unfortunately never clipped back in, went up and all of a sudden I'm on the ground and in pain. Jay meet floor, floor meet Jay."
